Registration form - Use for both credit & non-credit classes.

Drop form - For information on dropping a class, click here.  This form can be used only for courses registered through CPE. If you are a university student and would like to drop a university class, contact the Undergraduate or Graduate Registrar's office.

Credit Course Change

Pass/Fail - Audit Request

Change of Major (CE-UG students only) - To change to a different CPE Degree Program.

Credit Overload Petition - To request permission to take more than the maximum allowed CPE credits in a term. For information on credit limits, click here.

Independent Study Registration

Intent to Graduate (CE-UG students only) - Submit form prior to graduation along with the graduation fee.

Undergraduate Late Add Petition (Undergraduate students only) - To add a class after the add/drop deadline. Must also submit a Registration form. For more information on adding late, click here.

Graduate Late Add/Drop (Graduate students only) - To add or drop a course after the add/drop deadline. Must also submit a Registration form. For more information on adding late, click here. For more information on dropping late, click here.

Prior Approval for Transfer Course Work for Students in CPE Degree Programs - To transfer credit(s) from another institution to UMass Amherst in order to meet degree requirements. This form is not to be used by seniors who wish to complete any part of their senior year in absentia. For additional instructions, see page 2 of the form.

Transcript Request - To request a transcript of classes taken through CPE.  If applying to UMass Amherst: Note that the university’s Undergraduate Admissions and Transfer offices accept unofficial transcripts; however, the university’s Graduate Admissions Office requires official transcripts issued by CPE.

Undergraduate Reactivation - For students in a CPE degree program that have not enrolled in classes in the last two semesters (not including summer and winter). Form must be signed by program director or departmental representative.

University Undergraduate Level Instructor Permission (CE-UG students only) - To enroll in a university class that is full.

Non-Degree students: Must wait for the first day of classes before enrolling in university classes.

• Obtain all necessary signatures.
• Submit this form and a Registration form to the CPE Registration Office.

Registration must be completed by the add/drop deadline.

Undergraduate Withdrawal Request - To withdraw from a class after the withdrawal deadline (the semester midpoint; for exact dates, click here). Withdrawals are granted for extenuating circumstances only and supporting documentation is required. For more information, click here.

Instructor Verification for Withdrawal - Submit with Undergraduate Withdrawal Request. An e-mail from the instructor is acceptable. For more information, click here.

High School Student Permission

Appeal for Refund 

Medical Verification - Submit with Undergraduate Withdrawal Request. Note: Do not use this form for mental health verification. Instead, please contact the CPE Advising department at 413-545-3430.
