Electronic Course Drop Form

Use this form to drop classes electronically until the end of the Add/Drop period. For information on dropping a class after the Add/Drop period, click here.

Note: The Electronic Course Drop Form can be used only for courses registered through C&PE. If you’re a university student and want to drop a university class, please contact the Undergraduate or Graduate Registrar’s office.

Change of Major form sm_pdf_icon (CE-UG students only)

This form is to be used by students who are currently active in a C&PE Degree Program, but would like to change to a different C&PE Degree Program.

Credit Overload Petition pdf icon

Use this form to request permission to take more than the maximum allowed C&PE credits in a term. For information on credit limits and instructions, click here.

Undergraduate C&PE degree students: Submit this form prior to graduation along with the graduation fee.

Prior Approval for Transfer Course Work for Students in C&PE Degree Programs pdf icon

Use this form to transfer credit(s) from another institution to UMass Amherst in order to meet degree requirements. This form is not to be used by seniors who wish to complete any part of their senior year in absentia. For additional instructions, see page 2 of the form.

Transcript Request Form pdf icon

Use this form to request a transcript of classes you’ve taken through C&PE.

If you’re applying to UMass Amherst: Note that the university’s Undergraduate Admissions and Transfer offices accept unofficial transcripts; however, the university’s Graduate Admissions Office requires official transcripts issued by C&PE.

Undergraduate Reactivation Form pdf icon

Use this form if you’re a student in a C&PE degree program but have not been enrolled in classes in the last two semesters (not including summer and winter). The form must be signed by your program director or departmental representative.

University Undergraduate Level Instructor Permission Form pdf icon

Matriculated C&PE students: use this form if the university class is full.

Non-Degree students: you must wait for the first day of classes before enrolling in university classes.

• Obtain all necessary signatures.
• Submit this form and a registration form to the C&PE Registration Office.

Registration must be completed by the add/drop deadline.

Undergraduate Withdrawal Request Form pdf icon

Use this form to withdraw from a class after the withdrawal deadline (the semester midpoint; for exact dates, click here). Withdrawals are granted for extenuating circumstances only. Supporting documentation is required. For more information and instructions, click here.

Instructor Verification for Withdrawal pdf icon

Submit this form with your Withdrawal Request Form. An e-mail from the instructor is acceptable. For more information and instructions, click here.

Submit this form with your Withdrawal Request Form. Note: Do not use this form for mental health verification. Instead, please contact the C&PE Advising department at 413-545-3430.
