Registrations submitted by mail, fax or in-person must be received in the C&PE Registration Office by the last day of add/drop.
Note: If you’re using a tuition waiver, please submit a tuition remission form with your registration. For more information, click here.
Two methods:
1) Use online registration to create form:
- Go to Classes page.
- Select your classes.
- Use the Create Request for Printing Option
- Mail or bring your completed registration form to:
Records and Registration Office Continuing & Professional Education University of Massachusetts 100 Venture Way, Suite 201 Hadley, MA 01035-9430
OR
- Fax your completed registration form to: 413-545-0513
2) Use blank registration form:
- Select your class(es) frm the class listing pages or the printed catalog.
- Fill out a registration form:
- Credit class registration form here
- Noncredit class registration form here

- Make sure to choose a form of payment: Billing (via Bursar's Office), MasterCard, Visa, Discover, check, or money order (make checks and money orders payable to University of Massachusetts) or 3rd party billing.
- Mail or bring your completed registration form to:
